Two things make this work. Government hospital report. And reasonable new location request.
If it’s security, replace medical lines with security facts. Example: My PPA is in area with recent bandit activity reported by local police. Attach a police report or LGI confirmation if you have it.
How To Submit It
Submission differs by state. Some states are strict. Some are flexible.
In Lagos, you submit to your LGI at Agege or Surulere LGA office. They forward to State Coordinator at Alausa. You get a stamp.
